Depending on whether you are a sponsor, donor, provider, or patient, we may collect, use, store, and transfer different kinds of personal data about you which we have grouped together as follows:

  • Identity Dataincludes first name, middle name, maiden name, last name, username or similar identifier, date of birth, gender, tax ID/social security number, national practitioner identifier number, copy of provider license, and copy of government-issued photographic identification.
  • Contact Dataincludes billing address, mailing address email address, and telephone numbers.
  • Financial Data includes bank account and payment card details.
  • Transaction Data includes details about payments to and from you.
  • Technical Dataincludes Internet protocol (IP) address, your login data, browser type and version, time zone setting and location, browser plug-in types and versions, operating system and platform, and other technology on the devices you use to access this website.
